Scholarship Application

All information received for the purpose of scholarship evaluation will be held in strict confidence, and at the highest level of privacy. All evaluation forms will be destroyed following the evaluation process. No information of any kind will be shared with any other person, entity, or organization.

Process – Please submit this questionnaire prior to registration for your child’s sport, or simultaneously as you register your child for a given sport. If you register simultaneously, please check the box for TRYS that you’ll find upon the registration application. You will not be asked for the fee at the time of registration by checking this box while either registering in person, or online. 

Upon the completion of evaluations, the TRYS Foundation will inform you as to the degree of scholarship award. Should you accept the scholarship, the payment for the scholarship will be made in behalf of your youth athlete to Trussville Parks and Recreation. In cases where partial scholarships are awarded, and accepted, you will be responsible for paying the balance of the registration fee to Trussville Parks and Recreation.
